Build forms that sell, book, and sign.
Custom Contact Forms Pro turns the builder you already know into forms that take payments, collect signatures, autocomplete addresses, text confirmations, and send PDF receipts. No e-commerce stack required.
$80 per year
Checkout is handled by OIO Publisher, our parent company. You’ll see that name at checkout and on your receipt.
What will you build?
If it collects information, a signature, or a payment, Pro can build it. No checkout plugin, no cart, no e-commerce stack.
Donation Forms
Preset amounts plus a conditional “Other” box, with minimums enforced and optional fee-cover add-ons.
Product Order Forms
Products with images, quantities, coupons, and a live-updating total, charged right on the form.
Event Registrations
Multi-step signup with per-step validation, attendee details, and payment on the final page.
Deposits & Bookings
Take a service deposit and lock in the booking, with agreement, signature, and payment in one flow.
Invoice Payments
Let clients pay a fixed invoice or enter their own amount, with a Stripe receipt sent automatically.
Waivers & Agreements
Scrollable terms, an agreement checkbox, and a drawn signature. Every submission records the exact terms agreed to.
Fundraisers & Tip Jars
Customer-chooses-amount giving with enforced minimums and an optional cover-the-fees checkbox.
Surveys & Feedback
Likert grids, star ratings, and multi-step questionnaires with conditional follow-up questions.
Premium fields, ready to drag in
Every Pro field drops into the same drag-and-drop builder you already use, and conditional logic works on all of them, priced fields included.
Plus PDF, built in: payment receipt PDFs attach automatically to notification emails, and any submission downloads as a PDF from the admin, signature ink and PAID stamp included.
Don’t want to start from scratch? 13 ready-made Pro templates, from Donation to Rental Agreement, set these fields up for you in one click, complete with real starter legal text on the agreement forms.
Get paid on the form itself. No redirect, no checkout page.
Pro embeds the modern Stripe Payment Element directly in your form. Visitors pay where they are, and eligible devices automatically offer Apple Pay, Google Pay, and Link. Prefer PayPal? Add the PayPal field and buyers pay in place with their PayPal account, without leaving your page.
- Two gateways, one form. Stripe cards and wallets, PayPal, or both side by side
- Dynamic pay button. Reads “Pay $25.00” and live-updates as products, quantities, and coupons change
- Server-side verification. Every charge amount is recomputed and checked on your server before anything saves, so totals can’t be tampered with in the browser
- Receipts sent automatically. Stripe-branded emails from Stripe, plus your own PDF receipt attached to the notification
- No orphan charges by design. Payment is confirmed before the submission is stored, so a customer is never charged without a completed form
Fewer typos. Faster replies.
Two additions for forms that book jobs and take deliveries, where a wrong address or a slow confirmation costs you real money.
Address Autocomplete
Your visitor starts typing and picks their address from a list. Street, city, state and ZIP fill themselves in correctly, and in the right boxes.
- Powered by Google’s address data
- Limit suggestions to the countries you serve
- Every field still editable by hand
- If Google is blocked, the fields work as normal text boxes, so nobody is ever stuck
Uses your own free Google API key. A built-in test button checks it for you and says exactly what is missing if anything is.
SMS Confirmations
Text your customer the moment they book or pay. For deposit and appointment businesses, a text lands in seconds where an email waits in a spam folder.
- Write your message once, with answers from the form dropped in
- Send on every submission, or only once a payment clears
- Consent checkbox support, so you only text people who agreed
- Test mode sends nothing and costs nothing while you set it up
Uses your own Twilio account. US senders need Twilio’s A2P registration before texting US numbers. That is Twilio’s requirement, not ours, and our docs walk you through it.
